Forms (State of MN and Federal Forms)
If you are currently or will be a participant in the State of MN Tuition Grant/Reimbursement or State Cash Bonus and have not submitted your Direct Deposit Sign Up Form; you may download from here. As of 01 Jul 2008, it is mandatory to sign up. You will need to submit this form before any further Tuition or State Cash bonus payments will be made.
If you are an SLR participant, you must fill out and return the DD 2058, Certificate of Residency form before any SLR payments can be made. You may download that form below.
